Basic Service Plan
Care and Service Packages:
Each resident's care and services needs are determined through an assessment process conducted by a Registered Nurse. Care and service needs determine the Level of Care assisgnment. Each Care and Service Package includes all assessed needs. Residents/Responsible Parties will be notified if a Level of Care assignment changes based on assessed resident needs for care. Care and Service Packages do no include insulin management services.
There are 3 Levels of Basic Care and Service Packages.
There is also a Diabetes Management Package with includes weekly insulin set-up by a Licensed Nurse, dosing, monitoring, and recording.
Personal Care Packages:
Assisted Memory Support:

Resident cooperates in assisting with routine cares such as dressing, grooming, bathing, and toileting. Medication management. Resident’s behaviors may require occasional staff intervention.
Advanced Memory Support:

Resident may be incontinent of bowel and bladder. Resident requires direct assistance of one staff person with dressing, grooming, bathing, medication management and incontinence cares. Resident’s behaviors may require frequent staff intervention.
